Records Requests

Please be advised that Stacey Prince, PhD is Dr. Frank DeMarco's Professional Executor. Please be assured that your personal health record has been secured by her and will be handled in accordance with the requirements of the American Psychological Association professional standards, as well as the requirements of the WA Department of Health.

Records will be stored for 8 years in Therapist Helper, a cloud and software based platform. 

If you would like a copy of your health record to be transferred to another mental health service provider, please sign a release of information form and return it to the email address below.

Upon receipt of your authorization form, Dr. Prince will make the appropriate arrangements concerning your file. Should you have any questions in regard to this please do not hesitate to contact Dr. Prince directly via frankdemarcophd@gmail.com
